Guest Release Form

This form should be provided by all non-member

youth and adults sailing with Ship 1000.

Requires parent/guardian signature for

sailors younger than 18 years of age.

Class 1 Medical Form

The Class 1 Medical Form is a simple form listing

emergency contact information and any allergies, etc.

Anyone sailing with Ship 1000 should provide

this form to Catherine or Adam.

MEMBER FORMS:

Youth Membership Application

This form should be completed by any youth (age 14 to 20)

who would like to join Ship 1000 as a Member.

This form should be submitted to Catherine or Adam

with the $10 annual dues.

This form requires a parent signature for youth

younger than 18 years of age.

Class 3 Medical Form

The Class 3 Medical Form is required for all

Venture Scout groups due to the high-adventure

nature of our activities.

This form should be completed by all Members,

and requires a physical and doctor's signature.
